Russian/Ukrainian launch vehicles have quality issues

As Russia’s Deputy Prime Minister Dmitry Rogozin tries to emulate the reliability improvement of Western launch vehicles by making the nation’s quality control systems for Russian launch vehicles more independent, it cannot come too soon. As the list below shows, most of the failures since 1 January 2010 have been quality control or design/testing related and most involve rocket upper stages.
